An updated scientific review from Cochrane confirms that smokers using nicotine e-cigarettes achieve higher quit rates than those relying on nicotine replacement therapies (NRTs) such as patches, gums, and lozenges. The findings reinforce high-certainty evidence supporting e-cigarettes as an effective smoking cessation tool.

Clinical Evidence Shows a 61% Boost in Smoking Abstinence

Led by Nicola Lindson and Jonathan Livingstone-Banks from the University of Oxford, the review evaluated 80 randomized controlled trials involving 29,861 participants. Nine clinical trials were added to the updated analysis, expanding on data collected since the initial 2014 publication.

The consolidated data indicates that smokers using nicotine vapes showed a 61% higher rate of maintaining tobacco abstinence for at least six months compared to those using traditional NRTs. Serious adverse events occurred at comparable rates across both groups, supported by moderate-certainty evidence.

The Paradox: Tobacco Harm Reduction vs. Nicotine Addiction

Although vaping carries health risks, researchers noted that these hazards are far lower than the dangers of continued combustible tobacco use. However, the high efficacy of e-cigarettes introduces new challenges regarding long-term nicotine dependence.

"Addiction to nicotine-containing e-cigarettes can be strong due to the nicotine levels delivered, which can make weaning off vaping difficult," noted tobacco specialist Professor Ivan Berlin. He pointed out that while e-cigarettes effectively displace combustible tobacco, meta-analyses show that vapers are roughly half as likely to achieve complete nicotine abstinence compared to patch users.

The authors concluded that nicotine vapes may not suit every smoker, recommending that individuals consult healthcare professionals to determine the most effective cessation strategy for their specific needs.
