    A coalition of global health scientists and former World Health Organization (WHO) officials is urging governments to integrate tobacco harm reduction into their public health strategies. Citing comprehensive epidemiological data, experts argue that embracing lower-risk alternatives like e-cigarettes and heated tobacco could prevent over 100 million premature deaths worldwide by 2060.

    The Quantifiable Impact of Harm Reduction

    The push for risk-proportionate regulation is grounded in extensive clinical and economic modeling. The following table highlights the projected public health and economic benefits of transitioning smokers to non-combustible alternatives.

    According to the Global State of Tobacco Harm Reduction report, if just 20 percent of current smokers switched to smoke-free alternatives over the next 10 to 15 years, smoking-linked mortality could be halved by 2060. A joint report authored by prominent figures, including former WHO officials Tikki Pang and Derek Yach, emphasizes that policy inaction carries severe consequences. Embracing harm reduction alongside conventional measures could "roughly double the lives saved compared to current policies alone."

    The clinical efficacy of these alternatives is well-documented. A comprehensive Cochrane review found high-certainty evidence that e-cigarettes are more effective for smoking cessation than traditional nicotine replacement therapies. Cristine Delnevo, director of the Rutgers Institute for Nicotine and Tobacco Studies, noted that moving down the risk continuum to products like nicotine pouches is highly beneficial for public health.

    Economic and Global Policy Shifts

    At a recent summit organized by the International Association on Smoking Control & Harm Reduction (SCHORE) in Athens, 200 experts from 51 countries concluded that science must guide tobacco control. Furthermore, the economic benefits of this shift are massive. A study by Professor Christopher Cabuay estimated that if half of Filipino smokers transitioned to non-combustible alternatives, the Philippines could save $3.4 billion annually in healthcare costs and lost productivity.

    Verdict: A Necessary Strategic Pivot

    The scientific consensus is clear: abstinence-only tobacco control is insufficient to combat the 8 million deaths caused by combustible smoking each year. By actively encouraging lower-risk alternatives alongside traditional prevention measures, global policymakers have a quantifiable opportunity to drastically reduce both the mortality and economic burden of the tobacco epidemic.
